XMC V5 Mod Menu for GTA IV Original.
No need to open xbox or hotswap.

1. Download the zip file and extract it to your desktop (or an easy to access location)

2. Configure a USB Flash Drive of 8gb or more to your xbox 360 (or use a transfer cable with your hdd, your choice)

3. Use an Xbox 360 file transfer program to transfer the folders across. (The best one to use is Horizon)

4. Drag the file named '363F83FA495DD20C49CCED214A53F9F6' from the folder '00004000' into your HDD/USB and wait for the green bar at the bottom to finish.

5. Make sure that in the games section it says GTA IV and the file size is over 6GB, I can't remember the exact size.

6. Plug your USb/HDD back into your xbox and play from the GTA IV original disc and if it's worked then you should be able to use the menu!

Controls (part 1):

X + DPad down to open evades launcher 
Down to additional mods using the Dpad then A to select it and open mod slot 1.
Tap B until you are out of Evades Launcher and Press LB + RB to open the XMC v5 menu.

XMC v5 Controls (part 2):

Dpad up: scroll up menu
Dpad Down: Scroll Down menu
A: Select Option
B: Go back
